We collect only what we need to provide safe care:

• Contact details — name, phone number, email address and, where relevant, your mailing address.
• Personal health information — your health history, assessment findings, treatment notes and progress, collected during your appointments.
• Insurance and billing details — where you ask us to bill your extended health plan directly.
• Website information — anything you type into our appointment request form, plus anonymised analytics about how pages are used.

Your information is used to:

• Assess, plan and deliver your treatment.
• Book, confirm and remind you about appointments.
• Bill you or submit claims to your insurer, when you ask us to.
• Meet the record-keeping and reporting obligations that apply to regulated health professionals in Ontario.

We do not sell your information, and we do not use your personal health information for marketing.

We handle personal health information in accordance with Ontario's Personal Health Information Protection Act (PHIPA) and personal information in accordance with the federal Personal Information Protection and Electronic Documents Act (PIPEDA). We collect and use your information with your knowledge and consent, except where the law permits or requires otherwise. You may withdraw consent at any time, subject to legal and clinical record-keeping requirements.

We share your information only where it is necessary and permitted:

• With clinicians inside your circle of care at this clinic.
• With your insurer or benefits provider, when you ask us to bill directly or submit a claim.
• With another health provider (for example your physician), with your consent.
• Where required or permitted by law.

This website may use privacy-respecting analytics to understand which pages people find useful. Where analytics are enabled, IP addresses are anonymised and no personal health information is sent to any analytics provider. You can block cookies through your browser settings without losing access to any part of the site.

Records are stored securely, with access limited to staff who need it to do their job. This website is served over HTTPS. Clinical records are retained for the period required by the relevant Ontario regulatory college, after which they are securely destroyed.

You have the right to access your own records, to ask us to correct information you believe is inaccurate, and to ask questions about how your information is handled.
