How physiotherapy works in Ontario
Referrals, regulation, OHIP, insurance, WSIB and accident benefits — the rules that decide what you can book and who pays for it, explained without the jargon.
Physiotherapy in Ontario — the short version
In Ontario you do not need a doctor's referral to see a physiotherapist, because physiotherapy is a direct-access profession regulated by the College of Physiotherapists of Ontario. Treatment at private clinics is funded through extended health benefits, WSIB or motor vehicle accident benefits. OHIP funds physiotherapy only at designated Community Physiotherapy Clinics, for specific eligible groups.

Regulation and direct access
Physiotherapists in Ontario are regulated by the College of Physiotherapists of Ontario. Registration is public and verifiable, and the profession is direct-access — meaning a physiotherapist can assess you and decide on treatment without a physician referring you first. That responsibility cuts both ways: part of a proper assessment is recognising the presentations that need a doctor, imaging or an emergency department instead.
Referrals — the persistent myth
Ontario law does not require a referral to see a physiotherapist. What creates the confusion is that many extended health benefit plans write a referral requirement into their policy as a condition of reimbursement. The physiotherapist does not need it; your insurer might. It is plan-specific, and a five-minute phone call before your first visit settles it.
OHIP — narrower than most people expect
Publicly funded physiotherapy in Ontario is delivered at designated Community Physiotherapy Clinics, for defined groups: broadly people aged 65 and over, people under 20, and patients recently discharged from hospital after a related admission. Capacity is limited and waiting lists are common. Eligibility rules have changed over the years, so confirm your own position with ServiceOntario or a designated clinic rather than relying on any private clinic's summary.
Extended health benefits and direct billing
Most private physiotherapy in Ontario is funded through employer or private extended health plans, typically with an annual maximum, often covering a percentage rather than the full fee. Many clinics bill insurers directly so you pay only the remainder. If you and a partner both have coverage, coordination of benefits usually lets you submit the balance to the second plan — routinely overlooked, and it can substantially increase what you have available.
Workplace injuries and WSIB
Injuries at work run through the Workplace Safety and Insurance Board rather than your benefits. Approved physiotherapy under an accepted claim should not cost you out of pocket. The sequence matters more than anything clinical: report to your employer promptly, get the claim registered, and tell the clinic the claim number at booking so reporting starts on day one.
Motor vehicle collisions
Ontario operates a no-fault system for accident benefits, which means your own auto insurer funds your treatment regardless of who caused the collision. Treatment is delivered under a plan submitted to the insurer, and available funding depends on how the injury is classified. Symptoms commonly appear 24 to 72 hours after a collision rather than at the scene, and delays in being assessed make both claims and recovery harder.
